You will need the following information when making an appointment.

  • Insurance Name
  • Name of primary insurance holder
  • Primary holder's Scocial Security number and DOB
  • Name of primary insured's employer
  • Member ID and group # if any
  • Name of patient and DOB

Please Note that we will contact the insurance company before your appointment to verify eligibility and determine what type of service is covered. However, please be aware that payment from your insurance company can not be guaranteed and benefits are not determined until your claim is processed.
